What happens to your body after you quit smoking?

The short answer
Fast at first, then slowly, then for a very long time. The NHS puts your pulse starting to return to normal at 20 minutes, the carbon monoxide in your blood halved at 8 hours and down to a non-smoker's level at 48. Lung function improves by up to 10% between 3 and 9 months, and your risk of heart attack is halved after a year. What most timelines leave off: the slowest risks take ten to fifteen years, and lung cancer risk never comes all the way back down.

The first 48 hours are the part you can actually measure
Almost everything that follows a last cigarette happens out of sight. The first two days are the exception, and the NHS lists them plainly enough that you can check the first item against your own wrist.

Carbon monoxide is the line doing the work in that list. It is the one the NHS pairs directly with "your oxygen levels are recovering", and it is the only entry that comes with a hard end point rather than a direction of travel: two days, and that particular number is a non-smoker's number.
That overlap is worth naming, because it is where people conclude the whole thing is a lie. The repair is on one clock and withdrawal is on another: the NHS puts withdrawal symptoms at 3 to 4 weeks on average, longer for some people. Day two is simultaneously the fastest physical change on the list and one of the hardest days there is. Both things are true at once, and only one of them is a feeling.
After that, the change moves to your breathing
The next stretch is slower and vaguer, which is honest — the NHS gives it as ranges, not dates.
Between 2 and 12 weeks, blood is "pumping through to your heart and muscles much better because your circulation will have improved". Between 3 and 9 months, coughs, wheezing and breathing problems improve "as your lung function increases by up to 10%".
Read "up to" as what it says. Ten per cent is the top of the range, not the average, and it is recovery of function rather than a reset — structural damage such as emphysema is not on this list because it does not reverse. What people usually report first is more ordinary than a number: fewer stairs that hurt, and a morning cough that stops being the first event of the day.
The mood change is the one people refuse to believe
The most common reason to keep smoking is that cigarettes take the edge off, and that quitting will make an anxious person more anxious. The evidence points the other way.
In a 2014 systematic review and meta-analysis in the BMJ, Taylor and colleagues compared people who quit with people who carried on. Quitters came out lower on anxiety (standardised mean difference −0.37, across 4 studies), on depression (−0.25, across 10), on mixed anxiety and depression (−0.31) and on stress (−0.27), and higher on positive affect (+0.40). Their conclusion is the striking part: the effect sizes are "equal or larger than those of antidepressant treatment for mood and anxiety disorders".
Two caveats that the headline version drops. These were observational studies, so people who managed to quit may differ from people who did not in ways the analysis cannot fully remove. And the measurements come from follow-up, not from week one. Nobody is claiming day three feels good.
One year halves your risk. Fifteen is when the gap closes.
The NHS gives two long-range milestones: after 1 year, "your risk of heart attack will have halved compared with a smoker's", and after 10 years, "your risk of death from lung cancer will have halved compared with a smoker's".
Both say compared with a smoker's. That is the phrase every copy-paste version of this timeline drops, and it changes the meaning entirely: these are comparisons with the version of you who kept going, not with someone who never started.
Measured against never-smokers, the same curve takes much longer. A 2025 review in Global Heart, going back over the Framingham Heart Study data, reports that former heavy smokers only stop showing a significantly increased cardiovascular risk compared with people who never smoked after 10 to 15 years — well past the five-year mark that standard risk calculators have tended to assume.
The part the timelines leave out
Lung cancer is where the honest version diverges most from the poster. In the Framingham cohorts, Tindle and colleagues found that people who had quit within the last five years had a 39.1% lower lung cancer risk than those still smoking. Large, fast, exactly what you would hope for.
And then: compared with people who never smoked, risk was still elevated — a hazard ratio of 3.85 — more than 25 years after quitting. Just over 40% of the lung cancers that occurred in former smokers happened more than 15 years after they stopped.
That is not an argument for carrying on. Every line above still moves the right way, and the only comparison you actually get to make is with the version of yourself who kept smoking. It is an argument against reading the timeline as a countdown to a clean slate, which is how it is usually sold, and which sets people up to feel cheated at year ten.
When you stop matters more than any single milestone
The best answer to "is it worth it at my age" comes from the 50-year follow-up of British doctors published by Doll and colleagues in the BMJ in 2004. Men born between 1900 and 1930 who smoked cigarettes and kept going "died on average about 10 years younger than lifelong non-smokers".
Stopping bought that time back on a sliding scale: cessation at age 60, 50, 40 or 30 gained "respectively, about 3, 6, 9, or 10 years of life expectancy". Read it backwards and it is the more useful sentence — the number shrinks the longer you wait, and at no age in that study did it reach zero.
None of this needs to be memorised. It is worth knowing for one reason: the whole list is indexed on days, not on how the days felt. Hour 48 arrives whether you spent it calm or white-knuckled, and it counts the same either way. In the UK, local Stop Smoking Services are free, and the NHS will point you to the nearest one.

Is it too late to quit at 50 or 60?
No. In the 50-year follow-up of British doctors, stopping at 60, 50, 40 or 30 gained about 3, 6, 9 and 10 years of life expectancy respectively. The gain is smaller the longer you wait, and at no age in that study was it zero.
Why do I feel worse in the first week if my body is recovering?
Because two things run at once. The measurable repair starts within hours, while nicotine withdrawal is at its worst over the same days — the NHS puts withdrawal symptoms at 3 to 4 weeks on average, longer for some people. Feeling terrible on day two is not evidence that nothing is happening.
